MOC Service Eligibility

It is the intent of the Medical Outreach Clinic (MOC) to provide high-quality, primary-preventive care for high-risk, medically underserved patients within Macomb and St. Clair Counties.

MOC patients are those that do not have health insurance or income to pay for services.

To maintain the mission and goals of the MOC, while guarding against potential abuse of its services, the following procedure to determine eligibility will be in place effective January 1, 2005

Eligibility Determination

To identify that individuals who present on the MOC fall within the Government Poverty Income Level of 200% or less. Criteria will be based upon the following household income:

  • Wages/salaries
  • DSS income
  • Social security income
  • Supplementary state benefits
  • Pension and annuities
  • Veteran benefits
  • Income from self employment
  • Worker's disability income
  • Unemployment income
  • Prize, gifts or inheritance
  • Dividends and interest
  • Benefits provided instead of money, i.e. free rent

This information is needed for both the patient and their spouse to determine combined household income.

Applicants Responsibility

  • Provide a copy of latest Income Tax Forms for all household income, with licensed preparer's signature/stamp or copy of self-prepared forms with W-2's to substantiate income on tax form
  • Provide MOC staff with a least 2 paycheck stubs or unemployment income statements
  • Copy of Social Security card
  • Proof of residency (valid drivers license or picture ID with current address)
  • Proof of any other income

Provision of MOC Service Upon Completion Of Eligibility Determination

Individuals who meet the Government Poverty Level Income of 200% or less, and are a resident of Macomb or St. Clair County without insurance, will be eligible for services provided at the MOC, which includes primary care and available medications.


Eligibility will be based upon the following household income level of 200% the current poverty level of Michigan 2005.Size Of Family     Annual Gross Income (2005)

For family units with more than ten persons, add $6,800 each additional member.


There are instances when a patient will require services that cannot be performed at the MOC, i.e. x-ray, CT scans, lab work. These services will be referred to Mount Clemens Regional Medical Center.


For lab and x-ray services, patients can expect to be billed a nominal fee for interpretation.


Testing such as CT scans, stress test, bone scans, echocardiograms, ultrasounds, any nuclear medicine testing, i.e. thryroid scan with iodine uptake, patients will be required to submit financial information to the MCGH Credit Department prior to the test being performed to determine eligibility for charity care.
