Rehab Goal: Work with patients and their families to provide the highest level of independence following total joint replacement surgery. This includes mobility, self care, and safe return to home.
This program is designed to provide acute therapy care to patients who have had a
- Total Knee Replacement/Arthroplasty
- Total Hip Replacement/Arthroplasty
The program consists of:
- Physical and Occupational Therapy in both an individual and group setting.
- A “Total Joint Protocol” is used which is approved by your Orthopedic Surgeon
- Emphasis is on mobility so the patient spends less time in the hospital bed, and more time doing functional activity.
- Individual and group therapy is used to increase motivation, help develop a more positive attitude, and speed progress.

ABOUT THE PROGRAM
The Joint Express program is a treatment approach that combines pre-operative education, personalized pain management and advanced therapy techniques. Compared to traditional hip and knee replacement rehabilitation, the Joint Express program gets patients moving sooner and shortens recovery time. One of the special features that makes Joint Express so successful is the dedicated physical and occupational therapy professionals and nursing staff. Consistent communication and coordination of care between therapy and nursing staff results in better pain management and a quicker recovery.
REHABILITATIVE SERVICES
Rehabilitative Services plays a vital role in the program. PT and OT offer a combination of individual and group therapy in order to increase motivation, help the patients develop a more positive attitude and speed the recovery process.
A typical patient would receive the following Rehabilitative Services treatment plan:
Prior to Surgery:
- Several weeks prior to surgery, the patient is invited to attend an educational class taught by physical therapy, occupational therapy, nursing, and case management.
Post Surgery:
Day 1:
AM
- Patient receives an individual beside Physical Therapy evaluation. The PT teaches the patient and family how to move in bed, sit at the edge of the bed, transfer and ambulate with an assistive device.
PM
- Patient receives individual Physical Therapy at the bedside for therapeutic exercise, transfer training and ambulation.
Day 2:
AM
- An Occupational Therapy evaluation is performed at the bedside for equipment needs and self care activities.
- Patient continues gait training and ambulates to the Joint Express room for group therapy sessions with PT and OT for upper extremity and lower extremity strengthening and range of motion.
PM
- Continue with program of gait training, therapeutic exercise and self care activities.
- Encourage “coach” participation in therapy (The coach is a family member or friend who will assist the patient when they go home)
Day 3-4:
AM/PM:
- Continue gait training, stair climbing and group therapy sessions.
- Continued self care activities & toilet/shower transfers as needed with OT
- Equipment is ordered for discharge.

- Patients can support / encourage each other
- It is more fun and social

Overall, the program has been very successful in decreasing the length of stay for joint replacement patients by helping them become more functionally independent sooner. It provides better pain management and has produced increased positive customer feedback.
