As a participating physician for McLaren Health Plan, we would like to announce a new injectable prescription delivery service. MHP is implementing a Specialty Pharmacy Program through 4D, our Pharmaceutical Benefits Manager, and McKesson Specialty Pharmacy, in an effort to better serve you and our members.
McLaren Health Plan will implement the program starting January 1, 2005. The new service will eliminate the need to stock and bill for office administered injectable drugs by ordering patient-specific injectable and infused medications one dose at a time.
McKesson Specialty Pharmaceutical is an experienced specialty pharmacy with a proven record of high quality customer service and satisfaction, currently servicing over 40 million managed care members nationwide.
The process is very simple. When a patient needs an injectable medication, the prescription can be submitted by fax or phone. Specialized request forms can be obtained by calling 1-888-274-2031 or online at www.4dpharmacy.com and selecting the Physicians Corner. We have included forms with this communication.
McKesson will coordinate authorization with 4D and ship the prescription to your office. All packages are individually marked for each patient, and refrigerated drugs are shipped in insulated packages with frozen gel packs. This service also offers the additional convenience of enclosing all needed ancillary supplies (needles, syringes, and alcohol swabs) with prescriptions at no charge.

- Therapy support from specially trained pharmacists and nurses
- 24-hour Side Effects Hotline staffed by nurses
- Automatic coordination of monthly refills and delivery dates
